Athletics Participation Forms and Fees
Students in grades 6-12 who choose to participate on an ACS athletic team must complete and submit various forms to the Athletic Department to help ensure the safety and well being of every athlete. The forms address such topics as transportation of athletes, signs and symptoms of concussion, facts on sudden cardiac death, and granting staff and coaches permission to respond to any emergency medical needs.
Every athlete is required to have a sports physical on file before participating in competition. A Sports Physical Deferment Form is available for use for a limited time, but all athletes must have a physical completed by the end of the first month of each season — soccer (September), basketball and cheer (December), and baseball/softball (April). Forms must be completed by athletes and team managers participating in baseball, boys’ and girls’ basketball, cheerleading, golf, boys’ and girls’ soccer, softball, and track and field. The forms remain in effect for one year. There is also an Athletic Fee that must be paid for each sport in which a student participates, except for golf and track and field.
